*Your appointment is reserved just for you. If an appointment is not cancelled or rescheduled more than 48 business hours ahead of the scheduled start time, a Did Not Keep Appointment (DNKA) fee will apply. Appointments are expected to start on time and end on time. Patients 10 minutes or later for an appointment, or patients who do not arrive for their appointment, will be considered a missed appointment and charged as a DNKA. Stating that you did not receive a reminder does not exempt you from a DNKA fee.

Did not keep short follow-up appointment*: $250
Did not keep long follow-up appointment*: $500
**Please note that all appointments provided outside normal business hours (9AM-4PM Monday-Thursday) have an added $50 fee. After hours appointment/service is something we offer to accommodate those that are wanting to avoid disruption to their weekday schedules such as work or school. Based on availability, we provide appointments outside of our normal appointment hours of 9am-4pm Monday to Thursday. The fee is in addition to any other service/fee associated with the encounter.
***Please contact us if you have questions about which types of services are billed. Some examples include insurance related prior authorization (such as for medications), consultation with other professionals, completion of forms, summary preparation, letter preparation, clinical phone calls over 5 minutes, etc. Services will be billed on the basis of time expended at the hourly rate of $600. Please note that you will not be billed for phone calls related to scheduling, billing, or other non-clinical questions.
Fees are subject to change and reflect the complexity and type of service(s) provided. You will be notified thirty days in advance of any changes in our professional fees.
